You're Damp.

津液病 Jīn Yè Bìng — Phlegm and Dampness as disease substances.

Bloating, brain fog, heavy limbs, and unexplained weight gain aren't separate problems. In TCM they're one pattern with one root cause.

The weight that doesn't shift with diet. The brain fog that coffee doesn't clear. The bloating that comes and goes without obvious cause. This is Dampness — and it has nothing to do with how much water you drink.

Body fluids become pathological

Body fluids (津液 jīnyè) are the healthy liquid substances of the body — saliva, tears, synovial fluid, interstitial fluid. When the body's fluid metabolism is functioning correctly, fluids nourish and lubricate. When the metabolism is impaired — when the Spleen, Lung, and Kidney fail to transform and move fluids properly — they accumulate and thicken into pathological substances.

TCM describes two grades of pathological fluid accumulation:

  • Dampness (濕 Shī): Thin, diffuse fluid accumulation — puffiness, edema, that "heavy and waterlogged" feeling. Dampness tends to accumulate in the lower body and manifests as bloating, swollen ankles, loose stools, a sticky coat on the tongue.
  • Phlegm (痰 Tán): Thickened, congealed fluid — more concentrated than Dampness, with more diverse clinical manifestations. Phlegm can appear as visible mucus, but more importantly in TCM it refers to invisible accumulations throughout the body: the cyst you can feel under the skin, the brain fog that won't clear, the nodule that appeared on your thyroid.
The Phlegm You Can't See

"Tan" (痰) in classical TCM refers to two categories: visible phlegm (咳出之痰 — the phlegm you expectorate) and invisible phlegm (無形之痰 — phlegm you cannot see but whose effects you feel). Invisible phlegm can obstruct the Heart orifices (producing mental fog or even psychiatric symptoms), lodge in the joints (producing lumps, nodules, and stiffness), block the channels (producing numbness), or accumulate in the Liver (producing fatty deposits). The clinical dictum "a hundred diseases are born from Phlegm" refers specifically to this invisible, insidious category.

The five patterns of fluid pathology

Pattern 01 · Dampness Blocking the Middle (脾虛濕盛)

Core symptoms: Heavy, waterlogged sensation throughout the body. Bloating and fullness in the abdomen that doesn't change much with eating. Loose, sometimes urgent stools. Nausea. A sticky or sweet taste in the mouth. Thick, greasy tongue coat — the most reliable sign. The body feels heavy, movement feels effortful despite no muscular weakness.

"I wake up feeling swollen. I'm bloated all day regardless of what I eat. My legs feel heavy. My thinking is slow and thick. Nothing I do seems to clear it."

Er Chen Tang 二陳湯 — the foundational formula for resolving Phlegm-Damp. Dries Dampness, transforms Phlegm, harmonizes the Stomach.

Pattern 02 · Phlegm-Cold (寒痰)

Core symptoms: White, watery, or foamy mucus — copious, clear, and cold. Cold sensation in the chest or limbs. Dull ache in the joints. A preference for warmth. Fatigue with a cold, heavy quality. This is classic winter respiratory presentation — the chronic cough with profuse white phlegm that won't resolve.

Xiao Qing Long Tang 小青龍湯 for Lung involvement; warm-transform the Phlegm-Cold.

Pattern 03 · Phlegm-Heat (熱痰)

Core symptoms: Thick, yellow-green, viscous phlegm. Chest tightness. Restlessness and irritability — this is the pattern behind certain presentations of anxiety and insomnia where the mind feels "gunked up." Disturbing, vivid dreams. A red tongue with a thick, yellow, greasy coat.

Wen Dan Tang 溫膽湯 — clears Phlegm-Heat, especially when the Heart-mind is involved. The main formula for foggy, dream-disturbed insomnia with heat signs.

Pattern 04 · Phlegm-Damp in the Channels (痰阻經絡)

Core symptoms: Numbness or tingling in the limbs (without neurological cause). Stiff joints. Goiter or neck lumps (the thyroid is in the neck, governed by the Phlegm-obstruction pattern Ying 瘿). Subcutaneous nodules or lipomas. Globus sensation (Plum Pit Qi — feeling like something is stuck in the throat with no structural cause).

Ban Xia Hou Po Tang 半夏厚朴湯 — for Phlegm obstruction in the throat (Plum Pit Qi). Other formulas based on location and associated patterns.

Pattern 05 · Phlegm Misting the Heart-Mind (痰迷心竅)

Core symptoms: Mental fogginess that is qualitatively different from tired — a thick, heavy inability to think clearly, even after rest. Confusion. In severe presentations: sudden loss of consciousness, epilepsy, or psychiatric symptoms (see Heart Pathology for the full Phlegm-Cold/Phlegm-Heat psychiatric differentiation).

→ Depends on Hot or Cold type. Cold Phlegm: Su He Xiang Wan (蘇合香丸). Hot Phlegm: Meng Shi Gun Tan Wan (礞石滾痰丸).

Important

Unexplained weight gain, persistent edema (especially facial or lower limb puffiness), or new neck lumps/nodules should be medically evaluated before assuming a TCM Phlegm-Damp pattern. Thyroid disorders, kidney dysfunction, and lymphatic pathology can present similarly and require different management.
